Thuốc điều trị bệnh tuyến tiền liệt, giải pháp nào cho quý ông?

Thuốc trị bệnh tuyến tiền liệt ngày nay người ta có nhiều sự lựa chọn từ thuốc tân dược, liệu pháp hoocmone và cả thảo dược…

Tuyến tiền liệt là tuyến ngoại tiết thuộc hệ tiết niệu – sinh dục nam. Tuyến hình thành từ một túi mọc ra trên niệu đạo nam trong giai đoạn sớm của phôi thai. Ở người trưởng thành, tuyến tiền liệt nặng khoảng 15g nằm sâu trong hố chậu, ngay dưới bàng quang, nối với ống dẫn tinh và túi tinh.

Ngoài vai trò to lớn với sức khỏe sinh sản phái nam khi sản xuất ra tinh dịch để nuôi tinh trùng. Tuyến còn được ví như “người giám sát” trong cơ chế bài tiết nước tiểu ở niệu đạo. Tại đây, khi các chất được đào thải từ thận ra bàng quang sẽ đi qua tuyến tiền liệt trước khi được đưa ra ngoài qua ống niệu đạo.

Vì thế, bộ phận này có tác dụng lọc rửa một lần nữa các chất thải độc hại và xử lý chúng trước khi tống xuất ra ngoài, giúp cho hệ tiết niệu ở nam giới luôn khỏe mạnh, tránh được các nguy cơ như sỏi tiết niệu, viêm nhiễm… Tuyến tiền liệt có thể mắc nhiều loại bệnh từ tăng sinh đến viêm nhiễm thậm chí ung thư. Nhưng trong bài viết này chỉ đề cập tới thuốc điều trị phì đại lành tính tuyến tiền liệt – một căn bệnh hay gặp ở đàn ông tuổi trung niên.

Khi nam giới bước vào tuổi trung niên họ phải đối mặt với một căn bệnh gọi là phì đại lành tính tuyến tiền liệt và số người mắc bệnh này ngày một gia tăng. Bệnh phát triển ở vùng chuyển tiếp của tuyến, nguyên nhân gây bệnh có các giả thuyết bao gồm: Do nội tiết, do di truyền và hay do tương tác biểu mô – mô đệm… cho đến nay vẫn chưa ngã ngũ.

Chẩn đoán tình trạng bệnh dựa vào thang điểm quốc tế triệu chứng tuyến tiền liệt (IPSS) gồm: hội chứng kích thích, hội chứng tắc nghẽn, chỉ số chất lượng cuộc sống và kết quả siêu âm.

Với bệnh phì đại lành tính tuyến tiền liệt hiện nay phổ biến các thuốc điều trị như:

1. Thuốc ức chế thụ thể alpha 1 chọn lọc:

Thuốc có tác dụng ngắn phải dùng 2 lần trong ngày. Các loại thuốc được lựa chọn, bao gồm:

– Prazosin: Là dẫn chất quinazolin. Thuốc có tác dụng chính trên thụ thể alpha sau synap, làm giảm sức cản ngoại vi gây hạ áp. Thuốc còn có tác dụng giãn cơ trơn trong tuyến tiền liệt, do đó làm tăng lưu lượng nước tiểu ở bệnh PĐLTTTL. Tuy nhiên, thuốc này có nhiều tác dụng phụ. Cần thận trọng khi dùng phối hợp với thuốc lợi tiểu và ở bệnh nhân suy thận.

– Alfuzosin với biệt dược xatral viên 5mg. Riêng thuốc này có tác dụng kéo dài nên chỉ dùng 1 lần trong ngày.

– Terazosin với biệt dược hytrin, teranex, zonicat.

– Doxazosin với biệt dược carduran viên 2mg.

– Tamsulosin với biệt dược tamsustad viên 0,4mg.

Chú ý: không tự ý sử dụng nếu không được sự hướng dẫn của bác sĩ

2. Thuốc nội tiết tố

Hai loại: Progesteron và antiandrogen hiện nay ít dùng.

– Thuốc ức chế 5 alpha reductase (là men chuyển testosterone thành DHT trong tuyến tiền liệt). Việc giảm nồng độ DHT trong tuyến tiền liệt dẫn đến giảm thể tích tuyến và tăng lưu lượng dòng nước tiểu, cải thiện các triệu chứng tắc nghẽn đường niệu dưới, giúp bệnh nhân không cần phẫu thuật. Tác dụng không mong muốn của thuốc này liên quan tới chức năng cương, bất thường trong phóng tinh. Gồm: finasteride với các biệt dược finast, unibald viên 5mg và dutasteride với biệt dược avodart viên 0,5mg.

3. Thuốc thảo dược

Có thể thấy xu hướng dùng thảo mộc trong điều trị và hỗ trợ điều trị phì đại tiền liệt tuyến, u xơ tiền liệt tuyến đang ngày càng phổ biến. Thậm chí ở những nước có khoa học phát triển như Mỹ, các sản phẩm điều trị phì đại tuyến tiền liệt có nguồn gốc tự nhiên cực kỳ phong phú, người Mỹ ưa chuộng các sản phẩm này vì: Thứ nhất, phương pháp sử dụng tân dược chủ yếu để điều trị triệu chứng chứ không giải quyết được căn nguyên của bệnh và có nhiều tác dụng phụ. Thứ hai, kỹ thuật chiết xuất, bào chế đỉnh cao ngày nay cho ra đời những sản phẩm từ thảo dược nhưng hiệu quả rất thuyết phục.
