Phì đại tuyến tiền liệt lành tính hay U xơ tuyến tiền liệt là một u lành tính, thường xuất hiện ở nam giới tuổi ngoài 50. Bệnh có thể gây ra một số biến chứng khiến đường tiết niệu bị tắc.
Phì đại lành tính tuyến tiền liệt là gì?
Phì đại lành tính tuyến tiền liệt / u phì đại tuyến tiền liệt lành tính hay còn gọi là u xơ tuyến tiền liệt là chứng bệnh thường gặp ở nam giới tuổi trung niên trở lên. Tuyến tiền liệt phát triển trong suốt cuộc đời của nam giới. Khi mới sinh tuyến này có kích thước bằng hạt đậu, đến tuổi 20 – 25 thì ổn định và to cỡ quả mận. Tuy nhiên sau tuổi 40 tuyến tiền liệt có khuynh hướng phát triển bất thường. Chính sự to lên của tuyến tiền liệt đã gây ra các triệu chứng khó chịu ở đường tiết niệu.
Nếu không được điều trị, u phì đại có thể ngăn chặn dòng nước tiểu trong bàng quang và có thể gây ra một số vấn đề cho bàng quang, đường tiết niệu hoặc thận.
Nguyên nhân của phì đại lành tính tuyến tiền liệt
Nguyên nhân của tăng sinh lành tính tuyến tiền liệt cho đến nay chưa được biết rõ. Tuy nhiên có hai yếu tố quan trọng liên quan đến nguyên nhân sinh bệnh đó là: Tuổi tác và nội tiết.
– Tuổi tác: Nam giới trên 50 tuổi, nội tiết thay đổi : Testosteron(nội tiết tố nam) giảm, oestrogen (nội tiết tố nữ) tăng. Trong đó Oestrogen có tác động gián tiếp làm tăng tính nhạy cảm của thụ cảm Dihydrotestosteron (DHT) gây tăng sinh lành tính tuyến tiền liệt.
– Nội tiết: Hiện tượng phì đại lành tính tuyến tiền liệt xảy ra là do sự tương tác phức tạp giữa nội tiết tố nam Dihydroxytestosterone, một lượng nhỏ estrogen và các yếu tố tăng trưởng khác. Tạo nên sự mất cân bằng giữa sự phát triển và chết của tế bào, khiến tuyến tiền liệt lớn lên và phình ra.
– Giả thuyết khác: Một số yếu tố kích thích thường xuyên như: phóng tinh, tiểu tiện, viêm nhiễm…
Triệu chứng giúp nhận biết bệnh phì đại tuyến tiền liệt lành tính:
Khi mắc phải chứng phì đại tuyến tiền liệt lành tính / u xơ tuyến tiền liệt, bệnh nhân có thể gặp các triệu chứng sau:
- Tiểu khó: Tiểu tiện khó khăn, đứng rất lâu mới có thể đi tiểu được và cảm giác đi tiểu không có lực
- Dòng nước tiểu không thông suốt, ngắt quãng, tia nước tiểu yếu thậm chí không thành tia, đái nhỏ giọt
- Buồn tiểu không tự chủ, tiểu dắt và tiểu són vào cả ngày lẫn đêm khiến người bệnh đôi khi rất xấu hổ và khó chịu
- Đi tiểu nhiều lần trong ngày, đặc biệt là vào ban đêm
Một số người có tuyến tiền liệt phì đại lớn nhưng ít bị tắc nghẽn và ít triệu chứng. Lại có những người tuyến tiền liệt ít phì đại hơn, nhưng lại có thể bị tắc nghẽn nhiều và triệu chứng nặng hơn. Có khi người bệnh không nhận biết được sự tắc nghẽn đường tiểu cho đến khi đột ngột bí tiểu hoàn toàn. Hay còn gọi là bí tiểu tiện cấp tính, căn nguyên có thể do sử dụng một số loại thuốc cảm cúm có tác dụng ngăn chặn việc thả lỏng bàng quang để mở bàng quang và tống hết nước tiểu ra ngoài.
Phì đại tuyến tiền liệt lành tính mà nặng, theo thời gian, có thể gây ra những vấn đề nghiêm trọng như: nhiễm trùng đường tiết niệu, tổn thương bàng quang hoặc tổn thương thận, sỏi bàng quang và mất kiểm soát tiểu tiện. Khi bàng quang bị tổn thương thường xuyên thì việc điều trị chứng phì đại tuyến tiền liệt có thể không còn tác dụng. Chứng phì đại tuyến tiền liệt lành tính được phát hiện sớm ở những giai đoạn đầu, sẽ giảm nguy cơ phát sinh các biến chứng nguy hiểm cho bệnh nhân.
