Nguy cơ mắc viêm bàng quang mạn tính và cách phòng ngừa

Bệnh viêm bàng quang có thể gặp ở mọi lứa tuổi nhưng người trưởng thành chiếm tỉ lệ cao hơn cả. Viêm bàng quang cấp nếu không được chữa trị đúng có thể trở thành viêm bàng quang mạn tính, khó chữa và gây ra một số biến chứng nguy hiểm.

Bàng quang là gì?

Bàng quang là cơ quan chứa đựng nước tiểu thuộc hệ thống đường tiết niệu. Thành của bàng quang bao gồm các lớp cơ trơn và lớp niêm mạc cùng với mạch máu đến nuôi bàng quang. Cơ trơn thành bàng quang co bóp nhịp nhàng là nhờ được chỉ huy bởi thần kinh thực vật của não bộ.

Bàng quang có độ giãn rất tốt, ở người bình thường, bàng quang có thể chứa được 300ml nước tiểu hoặc nhiều hơn. Khi nước tiểu đầy sẽ sinh ra phản xạ buồn tiểu, lúc này thành bàng quang sẽ co bóp để tống nước tiểu ra ngoài, sau đó bàng quang co lại và tiếp tục lưu giữ nước tiểu từ thận xuống cho đến khi đạt giới hạn để thực hiện lần đi tiểu kế tiếp.

Nguyên nhân gây viêm bàng quang mạn tính?

Từ viêm bàng quang cấp nếu không được điều trị hoặc điều trị không đúng sẽ dẫn đến viêm bàng quang mạn tính. Viêm bàng quang có thể do nhiều nguyên nhân, trong đó cần lưu ý đến các nguyên nhân:

  • Vệ sinh cá nhân không tốt, các tác nhân gây ứ đọng nước tiểu nhất là ứ đọng nước tiểu ở bàng quang bởi sỏi, u, dị dạng bàng quang, lao, nhịn tiểu, ngồi lâu, táo bón lâu ngày hoặc sinh hoạt tình dục không an toàn.
  • Viêm bàng quang cũng có thể do tác động không mong muốn của các thủ thuật y tế như nội soi thăm dò bàng quang, thông bàng quang, dẫn lưu bàng quang, tán sỏi bàng quang hoặc phẫu thuật bàng quang.

Ngoài ra viêm bàng quang có thể là biến chứng của một số bệnh như: viêm, u xơ tiền liệt tuyến ( ở nam giới) gây ứ đọng nước tiểu ở bàng quang hoặc người mắc bệnh đái tháo đường, hẹp bao quy đầu…

Còn về căn nguyên của bệnh, viêm bàng quang chủ yếu là do vi sinh vật, trong đó vi khuẩn đóng vai trò đáng kể, nhất là họ vi khuẩn đường ruột, đứng hàng đầu là E.coli, sau đó là Proteus, Enterobacter, Citrobacter. Tiếp dến là vi khuẩn họ cầu khuẩn, chủ yếu là tụ cầu da hoặc tụ cầu hoại.

Đáng lo ngại nhất là viêm bàng quang do vi khuẩn mủ xanh (P. aeruginosa), bởi vì, vi khuẩn này có sức đề kháng rất tốt, đồng thời chúng có khả năng kháng lại nhiều loại kháng sinh.

Bên cạnh đó, viêm bàng quang còn có thể do vi khuẩn gây viêm niệu đạo cấp hoặc mạn tính đi ngược lên như Chlammydia, Mycolasma.

Triệu chứng của viêm bàng quang mạn tính

Viêm bàng quang mạn tính có biểu hiện gần giống như viêm bàng quang cấp tính nhưng không cấp tính rầm rộ và còn tùy thuộc vào mức độ năng hay nhẹ của bệnh.

Biểu hiện thường hay gặp nhất là đi tiểu nhiều lần, liên tục (do niêm mạc bàng quang bị kích thích bởi viêm nhiễm). Mót tiểu, tiểu buốt, tiểu rắt cũng thường gặp ở bệnh nhân viêm bàng quang mạn tính.

Một số trường hợp tiểu ra máu (do sỏi bàng quang hoặc lao bàng quang). Đa số viêm bàng quang mạn tính có tiểu ra mủ (nước tiểu đục và có mùi hôi). Bụng dưới thường có cảm giác tức, rất khó chịu bởi bàng quang ứ đọng nước tiểu. Khi người bệnh tiểu đục, có thể có sốt, tuy vậy, ở người tuổi cao, sức yếu, có thể không sốt.

Biến chứng nguy hiểm nhất của viêm bàng quang mạn tính là gây nhiễm trùng ngược dòng làm viêm thận (bể thận, đài thận), hậu quả có thể dẫn tới suy thận, tăng huyết áp.

Cách điều trị viêm bàng quang mạn tính

Muốn chẩn đoán chính xác bệnh ngoài các dấu hiệu lâm sàng, tiền sử bệnh, người bệnh cần được siêu âm hệ tiết niệu, nội soi bàng quang, chụp X-quang, tốt nhất là chụp cắt lớp hoặc chụp cộng hưởng từ Nếu có điều kiện nên nuôi cấy nước tiểu để xác định vi khuẩn gây bệnh, làm kháng sinh đồ để giúp bác sĩ điều trị chọn kháng sinh điều trị thích hợp hơn.

Khi nghi ngờ bị viêm bàng quang cần đi khám bệnh càng sớm càng tốt để được điều trị dứt điểm không để bệnh thành mạn tính. Khám bệnh giúp phát hiện nguyên nhân gây viêm bàng quang: do sỏi, u, lao hoặc do u xơ tiền liệt tuyến lành tính (nam giới) để tìm ra hướng điều trị dứt điểm từ nguyên nhân.

Cách phòng ngừa viêm bàng quang mạn tính

Muốn phòng viêm bàng quang mạn tính thì điều quan trọng nhất là không để mình bị viêm bàng quang cấp. Điều cần thực hiện là vệ sinh vùng hậu môn, bộ phận sinh dục ngoài sạch sẽ mỗi ngày. Không nên ngồi lâu, không nên nhịn tiểu. Giữ gìn trong quan hệ nam – nữ, nên vệ sinh sạch sẽ bộ phận sinh dục ngoài và đi tiểu trước và sau khi giao hợp.

Hàng ngày nên uống đủ lượng nước cần thiết (khoảng 1,5 – 2,0 lít) nên uống ít một thay vì uống cùng lúc lượng nhiều. Bổ sung thêm lượng nước từ rau và.

Chịu khó tập thể dục buổi sáng, đi bộ, chơi các môn thể thao (cầu lông, bóng bàn…).

Với người có tiền sử viêm bàng quang hay đang điều trị viêm bàng quang nên khám bệnh định kỳ.
