Viêm đường tiết niệu hay nhiễm khuẩn đường tiết niệu có thể gặp ở mọi lứa tuổi. Nhưng việc điều trị bệnh này ở người cao tuổi thì gặp nhiều khó khăn hơn vì sức đề kháng ngày một suy giảm.
Nguyên nhân gây Viêm đường tiết niệu
Ở người cao tuổi (NCT) do sức đề kháng ngày một giảm, thường mắc chứng sa sút trí tuệ (ảnh hưởng rất lớn đến sự điều hòa của thần kinh trung ương) khiến việc đi tiểu không kiểm soát được nên dễ bị viêm đường tiết niệu ngược dòng. NCT có thể bị nhiễm trùng đường tiết niệu cả phần trên (thận) và cả phần dưới (niệu quản, bàng quang, và niệu đạo).
- Nguyên nhân nội sinh thường gặp như: Sỏi đường tiết niệu, do cản trở dòng chảy của nước tiểu, lâu dần nước tiểu bị nhiễm vi sinh vật gây nhiễm trùng đường tiết niệu. Một số bệnh của tuyến tiền liệt, bệnh đái tháo đường, chấn thương cột sống, nằm lâu do bại liệt hoặc do bất động bởi cố định xương bị gãy (gãy xương đùi hoặc xương chậu hay gặp ở NCT).
- Nguyên nhân ngoại sinh gây nên viêm đường tiết niệu như: Nong niệu đạo, mổ lấy sỏi hoặc mổ u xơ tuyến tiền liệt bị bội nhiễm, viêm tuyến tiền liệt dẫn đến viêm đường tiết niệu.
Về căn nguyên gây nhiễm khuẩn cũng rất đa dạng nhưng chủ yếu là do vi khuẩn trong đó chiếm tỉ lệ cao nhất là E.coli. Ngoài ra, còn gặp một số trường hợp bị viêm đường tiết niệu do vi nấm.
Biểu hiện thường gặp của viêm đường tiết niệu
- Đau lưng có thể âm ỉ và xuất hiện từng lúc nhưng có khi đau thành cơn rõ rệt nhất là những lúc phải bưng bê, mang – xách vật nặng.
- Sốt và rét run cũng là một trong những triệu chứng thường gặp, tuy nhiên ở NCT có sức đề kháng kém thì sốt nhẹ hoặc không sốt mà chỉ thấy ớn lạnh.
- Đi tiểu nhiều lần, tiểu khó (buồn đi tiểu nhưng không tiểu được) và có thể bị tiểu đau, buốt.
- Màu nước tiểu có thể đục, có thể màu hồng (tiểu ra máu).
- Viêm đường tiết niệu do có vật cản (sỏi đường tiết niệu) thì thường kèm theo đau lưng hoặc có cơn đau quặn thận, có thể bị đái rắt, đái buốt…
Muốn chẩn đoán viêm đường tiết niệu thì ngoài các triệu chứng lâm sàng do bác sĩ khám bệnh khai thác và phát hiện được thì rất cần làm các xét nghiệm có liên quan như: siêu âm, chụp X-quang, xét nghiệm nước tiểu (tìm căn nguyên vi khuẩn bằng cách cấy nước tiểu đúng thường quy) mới hy vọng tìm ra căn nguyên gây nhiễm trùng của nó.
Điều cần làm khi bị nhiễm khuẩn đường tiết niệu
- Một biện pháp nên làm thường xuyên là vệ sinh cá nhân cho thật tốt. Vệ sinh đường sinh dục ngoài và xung quanh vùng sinh dục ngoài.
- Tập thói quen uống nhiều nước nhưng nên uống vào buổi sáng và chiều, cần hạn chế uống nhiều nước trước khi đi ngủ buổi tối để tránh đi tiểu đêm làm ảnh hưởng đến giấc ngủ.
- Không được nhịn tiểu bởi vì nhịn tiểu làm nước tiểu ứ đọng thời gian lâu sẽ gây bội nhiễm vi khuẩn và nguy cơ sẽ làm nhiễm trùng đường tiết niệu ngược dòng. Trước khi đi ngủ buổi tối nên nhớ đi tiểu.
- Không nên uống bia vào buổi tối hoặc trước khi đi ngủ buổi tối.
Nếu NCT mắc các bệnh như sỏi đường tiết niệu (sỏi thận, sỏi niệu quản, sỏi bàng quang…), các bệnh về tuyến tiền liệt thì cần khám bệnh để tìm cách xử lý vấn đề càng sớm càng tốt tránh ảnh hưởng dẫ đến viêm đường tiết niệu khi đã biết rõ nguyên nhân.
